| I, the undersigned, do hereby voluntarily submit my application
for participation in the 13th ANNUAL NORTHWEST INTER-CLUB KARATE TOURNAMENT
at Boeing Athletic Center, Everett, Washington, on October 11,
2003 and do hereby assume full responsibility for
any and all damages, injuries, or losses that I may sustain or incur, if
any, while attending or participating. I hereby waive all claims against
JAPAN KARATE FEDERATION RYOBU-KAI, or the officers and directors and management
thereof, or the
promoter, operators or sponsors, individually, or otherwise, for any damages,
injuries or losses that I may sustain. I fully understand that any
medical treatment given to me will be of a first-aid treatment type only. I
consent that any pictures furnished by me or any picture taken of me can
be used for publicity, promotion, or television showing
and that I waive compensation in regard there to. Further, I hold the
BOEING EMPLOYEE'S KARATE ASSOCIATION, JAPAN KARATE FEDERATION RYOBU-KAI,
MULTNOMAH ATHLETIC CLUB,
and all Tournament Officials and Judges harmless of any and all injuries. All
entries are final. NO REFUNDS will be given. |
